N2  -  The Cardiological Committee for Competitive Sports in its recent 2023 edition has issued indications for those who have undergone monovascular endomyocardial revascularization using a stent in the presence of atherosclerotic plaques causing a stenosis of 30% that show no more than two of the high-risk characteristics (fibrolipid density, micro-calcifications, positive remodeling). Regarding this, scientific evidence is not consolidated and expert opinions are not unanimous, which is why competitive sports fitness certificate could be granted based on an individualized assessment, according to the type of sport (II level of recommendation) and after performing an echocardiogram and ultrasound of the supra-aortic trunks, based on expert consensus and/or small retrospective studies and/or registers (C level of scientific evidence). These indications are not valid for those who had a stent implanted in the common trunk/bifurcation. Could in the other cases a morphological-functional examination such as the echocardiogram by physical exertion be appropriate in leading to a more accurate assessment of the sportsman asking for competitive fitness certificate?
