N2  -  The aim of this case report is showing how sports medical examination is important for unrevealing hidden pathologies, usually difficult to detect in asymptomatic subjects who practice sports of different competitive level. In some cases, moreover, the completion of the sports medical examination with the echocardiogram can be useful to point out clinical, congenital or genetic conditions that medical history and diagnostic tests required by our current reference legislation, cannot fully emphasize. In our case the young athlete was affected by bicuspid aortic valve and severe aortic coarctation associated with aortic insufficiency and left ventricular dilatation. The aid of diagnostic clinical tests such as echocardiogram, trans-esophageal echo and MRI should not however relieve the sports doctor from carefully visiting the patient, according to a useful and essential routine such as, inspection, palpation and in particular auscultation. In this specific case, the absence of femoral pulses, the presence of an evident systolic ejective murmur, should have already directed the physician to perform an echocardiogram.
